Incident Reporting

It is everyone's responsibility to report an incident, accident  or near miss when they have taken place and after the people involved have been made safe.iplanit

All incidents are now logged on to the new Electronic Incident and Accident Reporting System by those staff trained to do so.

  • A Manager and/or Senior Manager should be notified of the incident or accident within the required timescales; and

Support Staff and Housing Officers - all can directly log incidents on the Incident Management System and need to refer to and use the information below.

All other departments please complete the incident form below and email to the Incident Reporting email address for loading on to the new system.
